AIDS Drug Assistance Program
The purpose of the AIDS Drug Assistance Program is to ensure that underserved and uninsured individuals living with HIV/AIDS have access to life-saving medications.
IMPORTANT
The Florida Department of Health Bureau of HIV/AIDS has implemented cost containment measures for the AIDS Drug Assistance program –ADAP, as follows:
* EFFECTIVE JUNE 1st, 2010
WAITING LIST for new applicants
At least one (1) Antiretroviral (ARV) medication for new applicants
* EFFECTIVE AUGUST 1st, 2010
ADAP FORMULARY reduction (Antiretroviral meds & Opportunistic Infections ONLY)
Who will be placed on the waiting list?
* All new applicants
* Closed clients who did not pick up their medications within 60 days
* Clients with overdue enrollments

REGULAR APPLICATION PROCESS

- DOH Core Eligibility Application Form (click here for the FORM)
- DOH Consent to Fax Form (click here for the FORM)
- DOCUMENTATION
- Proof of HIV infection
- Proof of HIV residency
- Proof of Household Income < 400 % FPL
- No other access to medications through any other sources
- Prescriptions for at least one medication in the current ADAP formulary
- CD4 and Viral Load results, < 6 months old
- CASE MANAGER
